STEP 1 - Download the official application form from the Social Welfare Department website, or collect a printed copy from the local District Social Welfare Office or the office of the CDPO. The form is also available during the Meeyamgi Numit event held at the Chief Minister's Darbar Hall.

STEP 2 - Fill the form carefully, attach all the self-attested documents and include a recent passport-sized photograph.

STEP 3 - Submit the completed application in person at the office of the District Social Welfare Officer of your district before the last date announced for the year.
