Eligibility

An employee is eligible if they serve as a regular employee of the Government of Sikkim. Dependent family members are also covered, including the spouse, children, and parents, up to a total of 5 dependents.

Unemployed unmarried sons up to 30 years, unmarried daughters of any age, and unmarried unemployed children with certified disabilities are covered without age limits. Unmarried and unemployed brothers and sisters up to 30 years are also eligible as dependents.

Application Process

Online Application

STEP 1 - Visit the official Su-Swastha website and select Enrol Now.

STEP 2 - Login with your registered mobile number and complete the OTP verification.

STEP 3 - Fill out the two-part enrolment form carefully. Make sure you upload all required documents in the correct sizes.

STEP 4 - Agree to the declaration and submit the form. Your application moves to the concerned nodal officer for approval. Once approved, you can download your member e-card through the link sent to your email or mobile.

STEP 1 - Download and print the application form from the official portal.

STEP 2 - Fill in your details, attach a passport-sized photograph, and sign it.
