STEP 3 - Attach all the required documents, including the death certificate and the certificates from the Village Officer, and make sure they are self-attested where needed.

STEP 4 - Submit the completed application along with the documents to the Office of the Commissioner, Labour and Employment, and collect the official acknowledgment receipt for your records.
