The filled form, along with the required documents, is submitted to the same officer, who then forwards it to the District Medical Officer (DMO) after checking. The documents needed for this step are listed below.

• A copy of the delivery or discharge certificate given by the Medical Officer of the hospital, as proof of institutional delivery.
• A copy of the Mother and Child Protection (MCP) card.
• A copy of the birth certificate issued by the Registrar of Births and Deaths.
• A copy of the schedule tribe certificate of the father or guardian, or the domicile or residential certificate.
• A copy of the Aadhaar card of the parents or guardians.

Dulari Kanya Maturity Claim

When the girl turns 18, she has to submit a maturity claim application to the District Medical Officer. The claim is checked before the amount is released. The documents needed for the maturity claim are given below.

• A copy of the Class X passed certificate along with a copy of the mark sheet.
• A self-declaration about being unmarried on her 18th birthday.
• A copy of the bank passbook opened in the name of the girl beneficiary in State Bank of India.
• The withdrawal form filled by the girl herself.
