Karnataka government is collecting a printed enumeration form from every Gruha Lakshmi beneficiary in the re-verification drive that started on 30 August 2026. Survey staff hand over the form at home, the woman head of the family fills it on the spot, and it goes back with them the same day.
No official Gruha Lakshmi enumeration form PDF has been put up for download by the Women and Child Development department so far. Copies are printed and carried by the survey team, so you don't need to download or print one yourself. What the form asks is given below, so the details can be kept ready before the team visits.
Gruha Lakshmi enumeration form details
As per reports on the form shared by the government, the re-verification form collects around 10 details. Most of them match what was filled in the original Gruhalakshmi application form, so keep the same papers at hand.
- Ration card number of the family and the sanction letter number received after approval.
- Name and address of the woman head of the family.
- Her Aadhaar number and voter ID (EPIC) number.
- Her own bank or post office account, linked with Aadhaar and held at a branch inside Karnataka.
- Husband's name and his Aadhaar number.
- Category of the family: SC, ST, OBC or other.
- A working mobile number for contact.
Self-declaration part of enumeration form
Last part of the form is a signed self-declaration. She must state whether she or her husband pays income tax or files GST returns, and that she is not a government employee. She also declares that she is the head of the household.
By signing, she accepts that if any detail is found false, she can face legal action and will have to return the money paid under the scheme. Answer the tax and GST questions honestly. These answers are checked against tax records.
How enumeration form is filled at home
Enumerators are guarantee committee members, revenue inspectors, anganwadi workers and gram panchayat secretaries, assigned village-wise and ward-wise by the district administration.
STEP 1 - Survey team visits your house with a printed enumeration form and a mobile app that lists the beneficiaries of your village or ward.
STEP 2 - Fill the form with the details listed above and sign the self-declaration. If the beneficiary is not at home, another family member can fill it.
STEP 3 - Enumerator takes a photo of the beneficiary and of the house. GPS location of the house gets saved in the app on its own.
STEP 4 - Face authentication is done during the visit. If it fails, or the visit was missed, finish biometric verification at your ration shop or at the nearest Grama One centre, Karnataka One, Bengaluru One or Bapuji Seva Kendra.
