How to apply for a new job card at the Gram Panchayat
Only eligible families living in a rural area whose adult members are ready to do unskilled manual work can apply for the job card.
STEP 1 - Download the job card application form PDF given below and take a printout, or collect a copy from your Gram Panchayat office. Some states print the form in the local language, so use that one if your Panchayat has it.
STEP 2 - In the first table, write the name, age and gender of every adult in the house who wants work. Paste a recent passport size photo of each person in the photo column.
STEP 3 - Fill the household details: house number, village, Gram Panchayat, block, head of family, father or husband name, SC/ST/OBC category, BPL status, Aadhaar number, bank or post office account number and mobile number.
STEP 4 - Each adult listed signs or puts a thumb impression in the specimen column. Sign the declaration at the end and submit the form at the Gram Panchayat office. Ask for a dated receipt, as it proves when your 15 days started.
STEP 5 - Gram Panchayat checks that your family lives in the village and that the listed members are adults. After this, it issues the Gramin Rozgar Guarantee Card with a unique card number, photos of the registered adults, their bank or post office account number and Aadhaar numbers.

FAQ's
Can the Gram Panchayat cancel a job card?
Only in three cases: the card is a duplicate, the whole family has moved out of the Panchayat area for good, or all adult members of the family have died.
Our family had to move after a flood. Where do we apply?
During a declared disaster, displaced families can register and get a card from the Gram Panchayat or Programme Officer of the place where they are staying for now. Days worked there count towards the 125 days.
Do I need a separate card for each adult in the family?
No. One card is issued per household and it lists all registered adults. The 125 days of work are shared by the whole household.
