Application Process for Antim Sanskar Yojana
Apply through the Lok Seva Kendra, Janpad Panchayat office or the urban local body office. An online entry can also be made on the Labour Department portal using the worker registration number. Steps are given below.
STEP 1 - Keep the deceased worker's Board registration number and identity card ready, because every application starts with this number. Without a valid registration, the claim will not move ahead.
STEP 2 - Go to the Antyeshti and Anugrah application page on the Madhya Pradesh Labour Department portal, enter the worker registration number and the captcha code shown on the screen.
STEP 3 - Fill the details of the successor, that is the wife or husband, son, unmarried dependent daughter, or dependent parents, and enter the bank account details where the payment should come.
STEP 4 - Attach the death certificate, bank passbook copy and the FIR or postmortem paper in case of accidental death, then submit the form at the Lok Seva Kendra, Janpad Panchayat or Nagar Nigam office of your area.
STEP 5 - For rural areas, the Chief Executive Officer of Janpad Panchayat approves the case, and for towns and cities the Commissioner or Chief Municipal Officer approves it. After approval, the amount comes into the bank account by DBT payment.

Benefits Under the Scheme
Benefit is not limited to funeral money alone. Board pays a separate ex-gratia amount depending on how the worker died, and disability cases are also covered. Amounts as per current Board rules are given below.
