It is in Bengali and English and titled "কৃষক বсু (নতΦন) ϕকেџর έϠϏসমীϠা (Field Verification)-এর জনҝ যাচাইকরণ ফম κ / Form for field verification of Krishak Bandhu (New)". Your Block Agriculture Office also gives printed copies.
STEP 1 - At the top, write your Krishak Bandhu (KB) ID. You can find it on the farmer search page at krishakbandhu.wb.gov.in/farmer_search using your Aadhaar or Voter ID.
STEP 2 - In Part A (beneficiary details), write your name exactly as in your KB record or bank passbook, your father's, husband's, wife's or guardian's name, and your address with district, block, gram panchayat and mouza or village.
STEP 3 - Fill in your bank name and branch, IFSC and account number, the mobile number linked with Aadhaar, and your Aadhaar and EPIC numbers. The form also asks for the AC number, part number and serial number of your name in the voter list.
STEP 4 - Answer the Yes/No questions: whether your name was found in the ASDD list for the SIR/WBLA 2026 poll, whether a case is pending at the SIR Appellate Tribunal, whether a CAA application is pending, whether you get PM-KISAN, and whether you pay income tax. For primary occupation, write farmer, or state if you are a permanent employee of the central or state government, a local body or a government-aided institution getting regular salary or pension.
STEP 5 - Give your cultivable land details from your RoR: JL number, khatian number, plot (dag) numbers, area in acres, the RoR (khatian) date and how you got the land (purchase, death mutation, transfer or other). Sign below Part A.
STEP 6 - Fill Part B for your husband or wife: name, bank details, Aadhaar-linked mobile, Aadhaar, EPIC and voter list numbers, whether they are a Krishak Bandhu beneficiary, the same SIR and CAA questions, PM-KISAN and occupation. Sign the declaration, submit the form at the Block Agriculture Office and keep the tear-off acknowledgement slip the official signs.
By signing, you agree that the details are true, that false information can lead to cancellation of Krishak Bandhu benefits, and that the government can use your and your spouse's Aadhaar for verification. Farmers whose mobile number is correct in the Krishak Bandhu database are also getting an SMS about this step.

What the 3 September 2026 circular says
Directorate of Agriculture, Kolkata issued circular Memo No. 108/KBN/2026 on 3 September 2026 about checking, data entry, validation and approval of the returned verification forms. Its main instructions are below.
- District Deputy Directors of Agriculture (DDA) and ADA (Administration) supervise the work at block level, with extra staff or shift-wise work where needed.
- Supervisors randomly re-check 2% to 5% of approved cases by matching portal data with the physical forms. Each officer must check at least 500 cases in 10 days, and this was to be finished by 15 September 2026.
- DDAs visit blocks and look closely at rejected, cancelled, duplicate and doubtful records.
- The final decision to approve, include or exclude a farmer is taken by the ADA (Block) or DDA (Administration). The portal team only gives technical support.
- Farmers will be able to track the status of their form on the portal using KB ID or Aadhaar once that facility goes live.
- All preparations for DBT payment were to be ready by mid-September 2026, or as early as possible.
Krishak Bandhu verification last date
The Annexure-I form is dated 5 July 2026, and the first deadline was 30 July. It was extended past August. In late September, a new notification gave pending farmers 10 days to submit, so the window closes around the first week of October 2026. Rule changes to know before you fill the form
On 28 September 2026, Agriculture Minister Dudh Kumar Mondal said only one member of a family will now get Krishak Bandhu. Farmers with less than 40 decimal of land will get ₹4,000 a year and those with more will get ₹10,000. Farmers who join fresh will not get PM Kisan along with it, while existing PM Kisan farmers get ₹3,000 extra from the state. A formal order on these rules is not on the portal yet.
