Apply Process for Sarathi Shri Award

Whole process is offline. You collect the form, attach papers and hand over the file at your transport office. Steps are given below.

STEP 1 - First check that your Samagra ID is active. If your name is not on the Samagra records yet, register at samagra.gov.in or go to your nearest Lok Seva Kendra, MP Online Kiosk or Common Service Centre with your Aadhaar and address proof.

STEP 3 - Fill the form in clear handwriting. Write your licence number, vehicle details, years of driving and your bank account number exactly as printed in the passbook.

STEP 4 - Attach self attested copies of all papers and submit the file at your District Transport Office or Regional Transport Office. Ask for a receipt. Selection is done by a committee, and the award is given at a state level function.

Who Can Apply

• You should be a permanent resident of Madhya Pradesh.
• You should hold a valid commercial driving licence or a conductor licence issued in Madhya Pradesh.
• Your name should be registered on the Samagra portal with a family ID.

• Government employees cannot apply for this award.
