Application Form & Enrollment Process for Gujarat Karmayogi Swasthya Suraksha Yojana

Beneficiaries do not need to fill any separate application form for this scheme. State Health Agency does the enrollment automatically for all eligible employees and pensioners.

STEP 1 - Government employees and pensioners are directly included under the scheme by the State Health Agency.

STEP 2 - Each beneficiary family receives a special "G" series Ayushman Bharat PMJAY-MAA card.

STEP 3 - With this card, cashless treatment can be taken in government, equivalent and PMJAY-empanelled hospitals across the state.

STEP 4 - No extra application at department level is required, only the card is needed at the time of treatment.

For enrollment, the head of the office where the employee is serving issues a certificate showing the details of eligible dependent family members. For pensioners, this certificate is issued by the District Treasury Officer, Sub-Treasury Officer, Pension Payment Officer or Pay and Accounts Officer of the district from which the pension is drawn. Aadhaar number of each family member is also required for E-KYC.
