STEP 1 - Visit the local District Social Welfare Office and ask for the official application form for the scheme.

STEP 2 - Fill in all the required fields correctly. Paste a recent passport-sized photograph on the form and sign across it, then attach all the self-attested documents.

STEP 3 - Submit the complete file to the District Social Welfare Officer of your district.

STEP 4 - Always collect a receipt or acknowledgment for the application so the status can be tracked later.

Post-Submission - Once submitted, the District Social Welfare Officer reviews the request. If approved, the District Level Sanctioning Committee authorizes the purchase of the required aids from approved agencies for delivery to the beneficiary.
