  • Every Panchayat Bhawan in rural areas.
  • District-level selection centers in urban areas.

• Bank Passbook (first page copy)
• Self-declaration form (duly filled and signed by the applicant)
• Photocopy of the husband’s Aadhaar card (if applicable)
• Photocopy of the marriage certificate (if applicable)
• Birth certificate or school certificate as proof of age
