Form-6 is the new voter application form released by the Election Commission of India for getting a name included in the electoral roll. Any Indian citizen who is 18 years or above, or will turn 18 in a few months, can use this form to register as a new voter.
The form is available in Hindi and English on the official Voters' Service Portal at voters.eci.gov.in. It can be downloaded and filled offline, or filled online directly on the same portal and through the Voter Helpline mobile app. After submission, the application goes to the Electoral Registration Officer (ERO) of the assembly constituency where the applicant lives.
Details to be Filled to Apply as New Voter
Form-6 asks for basic personal details along with age and address proof. Every field should be filled carefully so that the application is not rejected. The main details required are given below.
- No. and name of the assembly constituency where the applicant lives.
- Name in the official language of the state, followed by the name in English in BLOCK LETTERS.
- Name and surname of any one relative (father, mother, wife, husband, legal guardian in case of an orphan, or Guru in case of a third gender applicant).
- Mobile number and email ID of the applicant, if available.
- Aadhaar details, or a note that the applicant does not have an Aadhaar number.
- Gender, date of birth, and a self-attested copy of any one age proof document such as a birth certificate, Aadhaar card, PAN card, driving license, Class X or Class XII certificate, or Indian passport.
- Present ordinary residence with the full postal address and PIN code.
- A self-attested copy of address proof in the name of the applicant or any one of the parents, spouse, or adult child. This can be a water, electricity, or gas bill of at least one year, a bank or post office passbook, Aadhaar card, revenue land records, a registered rent lease deed, or a registered sale deed.
- Category of disability, if any (optional).
- Details of a family member already included in the electoral roll at the current address.
- Declaration and signature or left thumb impression of the applicant.
After submitting the application at the nearest ERO, AERO, or BLO office, collect the signed acknowledgement or receipt. This receipt carries an acknowledgement number that can be used to check the application status later.

How to Apply Online for New Voter Registration
STEP 1 - Open the official Voters' Service Portal at voters.eci.gov.in and click on the "New Voter Registration" option on the homepage.
STEP 2 - Fill in the mobile number and complete the one-time password (OTP) verification to create a login.
STEP 3 - Select Form-6 and fill in all the required details like name, address, date of birth, and gender exactly as they appear in the supporting documents.
STEP 4 - Upload a recent passport-size colour photograph and self-attested copies of the age and address proof documents.
STEP 5 - Review the details, submit the form, and note down the acknowledgement number to track the application status.
