West Bengal government is inviting WB Khadya Sathi Scheme 2024 online registration / application form at wbpds.wb.gov.in. People can now apply online for Khadya Sathi Yojana at the official portal or through Duare Sarkar camps. The state govt. had launched WB Khadya Sathi Scheme on 27 January 2016. As per this scheme, 7 crore or almost 90% of the State’s population would get rice and wheat at Rs 2 per Kg. Around 50 lakh more would get the same at half the market price.
For the last few months, the State Government has been conducting the process of digitising ration cards. The digital ration cards replace the earlier ones. For the benefit of the people the Government of West Bengal has launched the campaign “Duare Sarkar” to provide various services under Khadyasathi (Corrections and updation in the existing Digital Ration Cards) to the doorsteps of the people.
The process is still ongoing. So those who have any issues regarding the ration cards, can log in to the website of the Food and Supplies Department of the Government of West Bengal at wbpds.gov.in
WB Khadya Sathi Scheme Registration / Application Forms
Here is the complete description of all types of WB Khadya Sathi Scheme Registration / Application Forms which must be filled to avail scheme benefits.
Application For adding left out family members of a family already having Digital Ration Card (DRC) [Form 4]
Collection of blank Form 4 & Submission of filed up Form 4
If few members of your family do not have Digital Ration Card, then application in Form 4 can be submitted. The application form is to be filled up properly and submitted with supporting documents. For getting the name of a family member left out, notified: Form IV-R / Form IV-U
Requirements for Khadya Sathi Form 4
- Photocopy of Digital Ration Card of the head of the family or any member (who has digital ration card)
- Photocopy of the applicant’s Aadhaar card if aged above 5 years( in case of age below five birth certificate)
- Photocopy of the applicant’s voter card (EPIC)
- Valid mobile number (Applicant should give his/her own / family member’s mobile number)
Application for correcting any information present in the Digital Ration Card [Form 5]
Collection of blank Form 5 & Submission of filed up Form 5
If any correction is required in the existing Digital Ration Card (such as spelling of name, address, date of birth etc.), then application in Form 5 can be submitted. The application form is to be filled up properly and submitted with supporting documents. For getting a name or address corrected: Form V-R / Form V-U
Requirements for Khadya Sathi Form 5
- Photocopy of the current Digital Ration Card
- Documents like photocopy of Voter Card (EPIC) / Kisan Credit Card / Bank Passbook / Driving License / Passport in support of required corrections.
- Photocopy of the Aadhaar of all the family members.
- Valid mobile number (Applicant should give his/her own / family member’s mobile number)
Application for change of ration Shop / Fair Price Shop [Form 6]

For change of Fair Price Shop (Dealer) / S.K.Oil Shop (K.Oil Dealer), application in Form 6 can be submitted. The application form is to be filled up properly and submitted with supporting documents. For getting changing to a new ration shop: Form VI-R / Form VI-U
Requirements for Khadya Sathi Form 6
- Photocopy of the current Digital Ration Card of the applicant(s) and head of the family
- Photocopy of the Aadhaar card of the applicant(s)
- Photocopy of the Aadhaar of all other family members.
- Photocopy of marriage certificate, if available (in case of shifting due to marriage)
Application for surrender of Digital Ration Card due to death or any other reason [Form 7]
Collection of blank Form 7 & Submission of filed up Form 7
For surrender of Digital Ration Card due to death or any other reason, application in Form 7 can be submitted. The application form is to be filled up properly and submitted with supporting documents.
Requirements for Khadya Sathi Form 7
- Original Digital Ration Card
- Photocopy of the Aadhaar of all the family members
- Photocopy of the death certificate / cremation certificate (in case of surrender due to death)
Application for duplicate Digital Ration Card [Form 9]

To get duplicate Digital Ration Card due to lost or damage, application in Form 9 can be submitted. The application form is to be filled up properly and submitted with supporting documents.
Requirements for Khadya Sathi Form 9
- Photocopy of Digital Ration Card (lost or damaged)
- GD number, if available (in case of lost or theft).
Application for General Category Card (Non Subsidized Digital Ration Card) [Form 10]

For new General category Digital Ration Card (Non-subsidized), application in Form 10 can be submitted. The application form is to be filled up properly and submitted with supporting documents.
Requirements for Khadya Sathi Form 10
- Photocopy of Ration Card, if available
- Photocopy of Aadhaar card of all members above 5 years
- Photocopy of birth certificate of the member(s) below 5 years
Application to link AADHAAR and mobile number with the Digital Ration Card [Form 11]

For seeding mobile number and Aadhaar number with the Digital Ration Card, application in Form 11 can be submitted. The application form is to be filled up properly and submitted with supporting documents.
Requirements for Khadya Sathi Form 11
- Photocopy of Digital Ration Card
- Photocopy of Aadhaar Card
WB Khadya Sathi Scheme Form 3 & Form 8
If the age of the applicant or age of family member is less than 5 years, then Aadhaar card is not mandatory, in that case Photocopy of birth certificate has to be submitted. All the above mentioned services are also available online and in the office of the Inspector (Food and Supplies) and Bangla Sahayata Kendra (BSK). If none in the family has Digital Ration Card, application in Form 3 can be submitted online and in the office of the Inspector (Food and Supplies).
For conversion of category from RKSY-II to RKSY-I or from General category to RKSY-I, application in Form 8 can be submitted online and in the office of the Inspector (Food and Supplies). Family who do not have any digital ration card may apply online in Form-3 or may submit application in Form-3 in the office of the area inspector (Food and Supplies). For getting a name notified for ration card: Form III-R / Form III-U
Similarly for conversion of categories of DRC (Form RKSY-II to RKSY-I or from General category(non-subsidised) to RSKY-I) the beneficiary may apply online in Form-8 or may submit application in Form-8 in the office of the area inspector (Food and Supplies).

- Then click on the menu button “CITIZEN” tab and select the appropriate sub-menu tab from the dropdown.
- Online & offline forms can also be submitted in the nearest Banglar Sahayata Kendra (BSK).
Those who have any enquiries regarding the getting of food grains through the public distribution system (PDS), that is, from ration dealers, can call up 1967 or 18003455505, from 8 am to 8 pm. Those who still have the old ration cards can, however, get non-PDS goods (there are 50 such items) and kerosene oil.
