- You should be a bonafide native of Uttar Pradesh.
- You should have been a member of the state senior team for three consecutive years and participated in National Games or National Senior Championships, while also earning a medal in the year you are recommended.
- You are considered eligible if you have participated in international events like the Olympic Games, World Championships/World Cups (held at 4-year intervals), Asian Games, Commonwealth Games, or Asian and Commonwealth Championships.
- You should provide an affidavit confirming you have never used banned drugs in any competition.
- You should submit a sworn declaration stating you have never been found guilty in any legal court case or regarding sexual harassment.
- You should be actively involved in any of the 31 recognized sports such as Swimming, Archery, Athletics, Badminton, Basketball, Boxing, Football, Hockey, Kabaddi, Cricket, or Wushu.
- This reward is given only once to an individual.
- Participation in National Senior Championships alone does not make you eligible.
- The award is also available to differently-abled sportspersons in the General Category and Veteran Category. The Veteran category is for sportspersons aged 40 years and above as of April 1 of the award year.
- The award can be given posthumously to Shahid sportspersons.
