UDID Card

Department of Empowerment of Persons with Disabilities, Central Govt. is inviting online applications for Unique Disability ID Card. Central Govt. will issue UDID Card and disability certificates to the Divyang (Viklang) people.

Candidates can even check and track their UDID Card Application Status and download e-Disability / e-UDID Card online. To avail the UDID Card, all the Persons with Disabilities (PWDs) will have to make online registration at swavlambancard.gov.in.

Apply Online for UDID Card / Disability Certificate

Unique ID for Persons with Disabilities is a flagship project of PM Narendra Modi Government. Govt. is implementing this project to create National Database and issue Unique Disability Identity Card for PwDs. Persons with Disabilities (PWDs) can Apply Online for the Disability Certificate / UDID Card. Before filling the online application, check who can apply and see the procedural guidelines.

UDID Card / Handicap Certificate Online Registration Form

This project will ensure transparency, efficiency and easy delivery of government benefits to Divyangjan. This UDID Card will also help in tracking the physical and financial progress of beneficiaries at Village, Block, District, State and National Level. Below is the complete procedure to apply online:-

STEP 1: Visit the official website swavlambancard.gov.in

STEP 2: On the homepage, click at “Apply for Disability Certificate & UDID Card” link or directly click https://swavlambancard.gov.in/Applyforudid

STEP 3: read the instructions, check the “I have read and understood the process” and then click on the Submit button as shown below.

STEP 4: Now select the first option for new cards or appropriate option as per your status from the next screen as shown below and click Submit.

STEP 4: Now Select “Apply with Aadhaar Number” if you have Aadhar number or if you have just applied for new Aadhaar number, then you can use Aadhaar Enrollment number as well, as shown in below image.

STEP 5: After entering your aadhar number and captcha code, click on Get OTP, verify the OTP received on Aadhaar linked mobile number and click Submit

STEP 6: In the next step, candidates have to fill all the necessary details accurately and upload “Photo” and “Signature” and click at the ‘Next’ Button. Then candidates have to enter the ‘Disability Details’, ‘Employment Details’ and ‘Identity Details’ as asked in the form.

STEP 5: Finally, click at the “Submit” button to complete the Person with Disability Registration process to apply for UDID Card / Disability Certificate.

The central government has launched the UDID app for tracking application status, download e-Card and other processes including the following.

Here people can search application by choosing their state / UT name, district name, either beneficiary number or Aadhaar number and enter beneficiary no. / aadhar no.

Swavlamban Card Home Login for UDID Card – Update Personal Profile

After application process, candidates can Update their Personal Profile and download e-UDID Card & Disability Certificate by making UDID Card Login.

Here handicapped persons can enter their enrollment number or UDID no., date of birth, captcha and then click at the “Login” button to make UDID Card Login.

All the candidates can apply online in case the UDID Card is lost. They just need to download and login into the UDID app from the link given above.

Track Application Status of UDID Card Online

Applicants can fill the UDID Card Application Form Online and can also apply through e-mitra or Atal Seva Kendra to get national disability identity card. Here candidates have to enter their “Enrollment Number / UDID Number / Mobile No. / Aadhaar Number” and click at SUBMIT Button to track their application status. After the application is approved, candidates can download their e-UDID Card & Disability Certificate by making Login.

UDID Card Benefits

The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:

  1. Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader
  2. The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future
  3. The UDID Card will also help in stream-lining the tracking of the physical and financial progress of beneficiary at all levels of hierarchy of implementation – from village level, block level, District level , State level and National level

Candidates can see the objectives of UDID Card Project. After getting UDID Card, all the disabled candidates will receive all the govt. schemes benefits in a transparent manner.

Important Points to Apply Online for Disability Certificate / UDID Cards

  1. Persons with Disabilities (PwDs) have already started applying online for Disability Certificate/UDID Cards from all States/UTs.
  2. Enrolment /Application Registration number will be generated for all the applications submitted successfully.
  3. SMS confirmation will be sent to the applicant with the enrollment/application registration number
  4. Applicants who do not have Disability Certificate  will receive SMS with date and time to visit the nearest CMO office for assessment
  5. The Disability Certificate /UDID Card will be generated online after assessment and verification
  6. SMS confirmation will be sent to the applicant with the allocation of UDID number
  7. The status of the application can be checked online by enrollment/application registration number
  8. Printed UDID Card will be sent to the PwDs on their Postal Address as and when printed (Process of printing the card is yet to start; will be intimated on the website).

What is a UDID card and who can apply for it?

The UDID (Unique Disability Identity) card is given by the Government of India to create a national database of persons with disabilities. Any Indian citizen with partial or full disability can apply online at www.swavlambancard.gov.in.

What documents are needed for UDID card and what size/format should they be?

You need a colored photo, Aadhaar details, address proof and disability certificate (if already available). Photo size should be 15-100 KB (jpeg, jpg, gif, png), signature 3-30 KB (jpeg, jpg, gif, png), address proof and disability certificate 10-500 KB (jpeg, jpg, png, pdf).

How many characters are in a UDID card number?

The UDID card number has 18 characters. These include state code, district code, CMO code, disability type, year of birth, running number and a check digit for security.

What is the validity of a UDID card and what to do when it expires?

UDID cards can be permanent or temporary. Permanent cards are issued when the disability is not likely to change, temporary cards are given when the condition may improve or worsen. If your temporary card expires, you must apply for renewal online through the PwD dashboard.

What are the types of UDID cards?

Cards are colour coded based on disability percentage: White (below 40%), Yellow (40-79%), and Blue (80% and above).

How is a UDID card generated and what guidelines are followed?

After online application, your form goes to the district medical authority. Doctors assess the applicant and the medical board finalizes details. Based on this, the card is generated and sent by Speed Post. Guidelines for assessment are available on the UDID and DEPwD portals.

How can I know about my assigned medical authority and what to do after assessment?

The details are shown on your UDID acknowledgement slip and also on the PwD login page. After assessment, you need to follow instructions of the assigned hospital/authority for further process.

How can I update or modify my UDID details? What if I already have a disability certificate?

You can update your application while it is in submitted mode by logging into PwD dashboard. Once the card is issued, details like name, gender, mobile and Aadhaar can be updated with approval. If you already have a disability certificate, choose “Yes” while applying. The authority may decide if reassessment is needed.

Is the UDID card linked to DigiLocker?

Yes, since October 2020 UDID cards and certificates are linked with DigiLocker.

Who issues UDID cards and what is the role of District Social Welfare Officer?

The issuing authority is the district’s Chief Medical Officer or equivalent. The District Social Welfare Officer handles schemes and benefits for PwDs at the district level.

How is the UDID card delivered and can it be issued in another district?

The card is delivered by Speed Post. Digital copies can also be downloaded from the portal. Normally, cards are issued from your home district or notified hospital where you are receiving treatment.

What are the benefits of UDID card?

The card serves as proof of disability to get government schemes, financial support, education, healthcare and job benefits. It also helps in easy identification and better accessibility services.

Whom should I contact if I don’t get a medical assessment call and how will I know my assessment date?

You should contact the CMO/DMO of your district. Normally, an SMS is sent with date and hospital details. If no mobile was given, approach the medical authority directly.

Is Aadhaar mandatory and how many disabilities are recognized?

Yes, Aadhaar details are required. Currently, 21 types of disabilities are recognized under the RPwD Act 2016.
